Regulated business entities in the field of medical devices receive a certificate of registration of the economic operator after the first successful registration at the JAZMP at the e-mail address provided in the application.
For each further certificate, the following applications shall be submitted:
Request for the issue of a certificate
Send the completed request for the issue of a certificate to the e-mail address or by post to JAZMP, Slovenčeva 22, 1000 Ljubljana.
The JAZMP will forward the digitally signed certificate of registration to the contact person at the e-mail address listed in the request for the issue of the certificate.
